 The following forms are for new clients; your therapist will let you know if they'd like you to fill these out.

For clients of : Carmen McHenry and Jennifer Kempema

(f you are clients of Florence Mancuso, Alicia Kiser or Victoria Ford - 

you will be emailed a link to fill out your forms. The email will come from "Simple Practice.") 

 Please print the forms and fill them out, and bring them with you to your first appointment. If your session is for a couple, each of you will fill out your own set of forms. If you are bringing in more than one child or teen, please fill out a set for each one. There are several pages. If you prefer to wait and fill them  out at the session, please arrive 15 minutes early for your first  appointment and let your therapist know, so that the forms will be ready  for you.
